Form Directions: Complete the Visitor Documentation form during a visit from anyone including but not limited to DCF inspectors, school staff, YMCA staff, and snack program inspectors.

Photo ID: Upon entrance to your site, please be sure to ask each visitor for a photo ID and obtain a business card. Should a visitor not provide proper identification, do not allow them access to the site. Call your supervisor immediately.
